Early Symptoms of Ectopic Pregnancy and When to See a Doctor

Approved By: Dr. Aparna Patil January 13, 2026 | 10:06 am

Pregnancy occurs when the fertilized egg attaches itself to the uterine wall. However, sometimes the fertilized egg can attach itself to other parts of the Uterus, such as the fallopian tubes, Ovary or the cervix. This is known as an ectopic pregnancy.

What happens during an extrauterine pregnancy?

When the fertilized egg abnormally implants itself in parts of the Uterus other than the uterine lining, it is called an ectopic pregnancy. Typically, it occurs in the fallopian tubes, though implantation in other parts of the Uterus is also possible. When the fertilized egg implants in the fallopian tubes, doctors refer to it as a tubal pregnancy. About 90% of extrauterine gestations happen in the fallopian tubes. The growing Embryo in the fallopian tube may rupture the tubes, causing severe bleeding and Shock. Therefore, early detection and treatment for abnormal pregnancy implantations is crucial.

Symptoms of ectopic pregnancy

A woman with an abnormal implantation will have early symptoms of pregnancy, such as:

  • Missed period
  • Nausea
  • Breast tenderness
  • Positive pregnancy test

However, an abnormal implantation cannot continue and must be removed immediately. So, how do you identify the early signs of an extrauterine gestation?

Vaginal bleeding

One of the first signs of a non-uterine pregnancy is vaginal bleeding. The vaginal bleeding may be spotting or light bleeding.

Abdominal pain

You might experience a sharp and acute pain in the abdominal or pelvic area. The pain may worsen, warranting immediate medical attention.

Shoulder pain

If the fallopian tube ruptures, you may also experience shoulder pain or an urge to have a bowel movement. Internal bleeding may irritate nerves that cause pain in the tips of your shoulders.  However, the type of pain depends on where the abnormal gestation takes place and where the blood collects.

Dizziness and weakness

Another common symptom of an ectopic pregnancy is feeling lightheaded, dizzy and weak.

What causes ectopic pregnancy?

As mentioned, almost 90% of extrauterine gestations occur in the fallopian tubes. So, what causes it? Any condition that slows the movement of the fertilized egg through the fallopian tube can cause this abnormal pregnancy. For example, infection, or an irregularly shaped fallopian tube and previous surgery on fallopian tube can obstruct the movement of the fertilized egg, causing it to attach to the tube instead of the uterine wall.
